The Opportunity
We are seeking an experienced Registered Nurse Case Manager to provide skilled home health services to patients across San Mateo and/or Santa Clara Counties. This is a flexible, field-based role with competitive compensation offered on a per visit or hourly basis, plus the option for full benefits.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans
- Coordinate and manage all aspects of patient care across the care continuum
- Perform skilled nursing visits including wound care, medication management, and patient education
- Collaborate with physicians, therapists, and other care team members
- Ensure accurate and timely documentation in compliance with Medicare/Medicaid standards
- Monitor patient progress and adjust care plans accordingly
Requirements
- Active California RN license
- Minimum 2 years of clinical nursing experience; home health experience preferred
- Strong knowledge of Medicare home health guidelines
- OASIS experience a plus
- Excellent time management and organizational skills
- Reliable transportation and valid California driver's license
- Compassionate, self-motivated, and comfortable working independently in the field
