    The Opportunity

    We are seeking an experienced Registered Nurse Case Manager to provide skilled home health services to patients across San Mateo and/or Santa Clara Counties. This is a flexible, field-based role with competitive compensation offered on a per visit or hourly basis, plus the option for full benefits.


    Responsibilities

    • Conduct comprehensive patient assessments and develop individualized care plans
    • Coordinate and manage all aspects of patient care across the care continuum
    • Perform skilled nursing visits including wound care, medication management, and patient education
    • Collaborate with physicians, therapists, and other care team members
    • Ensure accurate and timely documentation in compliance with Medicare/Medicaid standards
    • Monitor patient progress and adjust care plans accordingly


    Requirements

    • Active California RN license
    • Minimum 2 years of clinical nursing experience; home health experience preferred
    • Strong knowledge of Medicare home health guidelines
    • OASIS experience a plus
    • Excellent time management and organizational skills
    • Reliable transportation and valid California driver's license
    • Compassionate, self-motivated, and comfortable working independently in the field
