    Job Title: Registered Nurse – Home Health

    Location: Chisago City, MN
    Coverage Area: White Bear Lake, Stillwater, Vadnais Heights
    Employment Type: Full-Time
    Schedule: Monday–Friday | 8:00 AM–5:00 PM
    On-Call Requirement: Yes
    Compensation: Estimated Annual Earnings: $87,000–$105,000
    Bonus: $10,000 Sign-On Bonus

    Position Overview

    Wheeler Staffing Partners is seeking a compassionate and patient-focused Registered Nurse – Home Health for a full-time opportunity serving patients throughout the White Bear Lake, Stillwater, and Vadnais Heights areas. This role is responsible for delivering skilled nursing care in the home setting while coordinating individualized treatment plans and supporting positive patient outcomes.

    This position is compensated on a per-point basis, and the annual compensation listed reflects estimated annual earnings based on expected productivity. The ideal candidate will thrive in an autonomous environment, demonstrate strong clinical judgment, and be committed to delivering exceptional patient care.

    Key Responsibilities

    • Deliver skilled nursing care to patients in their homes in accordance with physician orders and care plans

    • Perform comprehensive patient assessments and identify healthcare needs

    • Develop, implement, and manage individualized patient care plans

    • Coordinate patient care with physicians, therapists, and interdisciplinary healthcare teams

    • Monitor patient progress and revise treatment plans as appropriate

    • Educate patients and caregivers on disease processes, medications, treatment plans, and preventive care strategies

    • Maintain accurate, timely, and compliant patient documentation

    • Support patient independence, safety, and quality of life initiatives

    • Participate in on-call rotation and respond to patient care needs as required

    • Travel throughout assigned service areas to conduct patient visits

    Qualifications

    • Graduate of an accredited school of professional nursing

    • Active Registered Nurse (RN) license in the state of practice

    • Minimum of one (1) year of RN experience required

    • Home health experience preferred but not required

    • Strong assessment, care coordination, and communication skills

    • Ability to work independently and manage a patient caseload efficiently

    Required Licensure & Certifications

    • Current Registered Nurse (RN) license in the state of practice

    • Current CPR certification

    • Valid driver’s license

    • Reliable transportation and current automobile insurance

    • Ability to travel throughout assigned coverage areas

    Work Environment

    • Primarily community and patient home-based environment

    • Frequent local travel required within assigned territory

    • Independent field-based role with collaboration across clinical teams

    • Participation in on-call rotation required
