Location: Baton Rouge, LA
Description:

Home Health RN – Full-Time 

Territory: 50 mile radius of Baton Rouge  70810

Schedule: Monday–Friday, 8:00 AM–4:30 PM 

Weekend Rotation: Yes TBD

Weekly Guarantee: 36 hours 

Mileage Reimbursement: Yes — reimbursed between visits (first patient to last patient) 

About the Role

Our client is seeking an experienced Hospice Registered Nurse to join our compassionate interdisciplinary team. This role is ideal for a nurse who thrives in a field‑based environment, values autonomy, and is committed to providing exceptional end‑of‑life care.

Key Responsibilities

• Complete IDG notes, recertifications, and admissions

• Provide daily patient visits and ongoing clinical assessments

• Deliver high‑quality, patient‑centered hospice care in home and facility settings

• Collaborate with the interdisciplinary team to support patient and family needs

• Maintain accurate and timely documentation in accordance with agency standards

Required Qualifications

• ASN (Associate of Science in Nursing)

• Hospice experience required

• HCHB (Homecare Homebase) experience preferred

• Strong clinical assessment skills

• Ability to work independently in the field

Preferred Skills

• Additional hospice experience beyond minimum requirements

• Strong communication and organizational abilities

• Compassionate, patient‑focused approach to care
