Job Title: Registered Nurse - Home Care

Department: Hudson Home Care

Location: ElderONE - Hudson Pace Center

Hours Per Week: 40

Schedule: Monday - Friday, 8:00 AM - 4:30 PM

SUMMARY

As a Registered Nurse in Home Care, your exceptional ability to teach, counsel and empathize with each patient leads to an enhanced quality of life.

RESPONSIBILITIES

• Patient Care

  • Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care.

• Communication

  • Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problems.

• Evaluation & Documentation

  • Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staff.

REQUIRED QUALIFICATIONS

• Nursing Diploma required • License and current registration to practice as Registered Professional Nurse in New York State • Current NYS Drivers license

PREFERRED QUALIFICATIONS

• ASN preferred • Bachelors degree of Science in Nursing preferred • 1 year of experience working with a frail or elderly population preferred • Availability of Motor Vehicle

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant.
