Position Overview
An established home health organization is seeking a Registered Nurse to provide individualized patient care in a home-based setting. This role offers the opportunity to work closely with patients one-on-one, develop meaningful relationships, and deliver comprehensive nursing care in a comfortable and familiar environment.
The RN will be responsible for assessing patient needs, developing and updating care plans, coordinating services with healthcare providers, and ensuring high-quality clinical outcomes while maintaining compliance with regulatory and organizational standards.
Key Responsibilities
Conduct comprehensive patient assessments and ongoing evaluations in the home setting.
Develop, implement, and revise individualized nursing care plans.
Ensure care delivery aligns with physician orders and approved visit frequencies.
Perform medication reconciliation and collaborate with physicians regarding patient care plans.
Deliver nursing treatments and procedures according to established clinical standards.
Document assessments, interventions, care coordination activities, and clinical decisions accurately and within required timelines.
Educate patients and caregivers using appropriate teaching methods to support treatment goals.
Coordinate care with interdisciplinary team members and facilitate referrals when appropriate.
Communicate changes in patient condition promptly to physicians and nursing leadership.
Manage insurance authorization requirements and maintain compliance with payer guidelines.
Supervise home health aides and complete required supervisory visits within regulatory timeframes.
Support continuity of care through effective communication during patient care transitions.
Meet productivity expectations while maintaining quality patient outcomes.
Stay current with evolving clinical standards and incorporate best practices into patient care.
Perform additional duties as assigned.
Work Environment
Home health setting serving patients throughout Ocean County.
Independent field-based role with regular patient visits.
Physical requirements include lifting, pushing, and pulling up to 50 pounds and prolonged standing and walking throughout the workday.
Required Qualifications
Associate Degree in Nursing or Nursing Diploma.
Active Registered Nurse (RN) license in the State of New Jersey.
Minimum of six (6) months RN experience or at least one (1) year of LPN experience.
Experience in home health or hospice is beneficial.
Basic computer proficiency.
American Heart Association (AHA) Basic Life Support (BLS) for Healthcare Providers certification.
Preferred Qualifications
Bachelor of Science in Nursing (BSN).
Additional home care experience.
