Schedule

Hybrid Position - 10% onsite, 90% remote
40 hrs a week, Monday Friday 8:00 am - 5:00 pm. No Weekends or Holidays

The Registered Nurse will complete a report for each member in the required format, documenting the medical necessity for skilled services to meet individualized needs. In-person assessments are required every 90 days and as needed. This position is field-based, and travel is required to complete these assessments. Responsibilities also include completing nursing referrals, placing vendor calls, and handling special projects as needed. Timely documentation is essential, along with interprofessional collaboration both internally and externally. Participation in multidisciplinary planning meetings is required to determine and identify appropriate services as per established policies and guidelines.

Duties

  • Exhibit excellent communication skills, including the ability to explain complex information and educate patients and families.
  • Collaborate with members and their support systems.
  • Attend mandatory meetings.
  • Complete required annual trainings.

Requirements

  • Registered Nurse licensed to practice in the relevant jurisdiction.
  • Recent experience in caring for individuals with medical feeding tubes, including gastrostomy tubes.
  • Minimum of 4-5 years of recent RN experience in home health, hospice, long-term care, or case management.
  • Additional training or certification in behavioral health, complex medical conditions, or nursing case management is preferred.
  • At least 2 years of experience working with individuals with disabilities.
  • Valid Arizona driver's license.
  • Arizona fingerprint clearance card required.
  • Moderate to advanced experience with Microsoft Word, Google Suite, and Adobe.
