How you'll make an impact in this role:
  • Lead the strategic direction and oversee the daily operations for a critical, high-impact case management program.
  • Drive high-quality patient outcomes by evaluating services, driving the development ofpersonalized care plans, and achieving performance excellence.
  • Collaborate with physicians and a diverse multidisciplinary team to champion evidence-based care and reduce clinical variation.
  • Serve as the go-to case management expert, aligning resources and specialized knowledge to support key, system-wide health-care initiatives.
  • Resolve complex operational challenges, ensuring a smooth and efficient work-flow for the entire case management department.
What minimum qualifications you'll need:

Licensure / Certification / Registration:

  • Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required.
  • Registered Nurse obtained prior to hire date or job transfer date required.

Education:

  • Bachelor's degree in nursing or diploma from an accredited school/college of nursing required. Master's degree or combination of Bachelor's degree and experience preferred.

Work Experience:

  • 5 years of experience required.
  • 10 years of experience preferred.
  • 2 years of leadership or management experience required.
  • 5 years of leadership or management experience preferred.
What additional requirements you'll need:
  • Minimum of 5 years of Case Management experience within an Acute Care setting
  • Minimum of 2 years of proven leadership / management experience in an Acute Care setting
