    How you'll make an impact in this role

    • Lead the implementation and continuous monitoring of individualized care plans to ensure every patient receives safe, evidence-based treatment.

    • Coordinate multidisciplinary teams as the primary point of contact, ensuring seamless communication across all specialties for holistic patient recovery.

    • Advocate for patient safety by accurately interpreting physician orders and communicating real-time clinical changes to the care team.

    • Navigate the discharge planning process in collaboration with the healthcare team to ensure patients have a clear, supported path for their transition home.

    • Champion clinical excellence by delivering direct nursing care that adheres to the highest organizational standards and safety protocols.

    What minimum requirements you'll need

    Licensure / Certification / Registration:

    • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
    • Licensed Registered Nurse credentialed from the Texas Board of Nursing required.

    Education:

    • Clinical Qualifications & Experience: Holds an active Registered Nurse license with background in Internal or Family Medicine, alongside hands-on proficiency using the Athena EMR platform.

    • Core Competencies & Capabilities: Demonstrates strong customer service, problem-solving, and multitasking abilities, with the independence to work autonomously and flexibility to travel as needed.
