    Registered Nurse Case Manager – Hospice | Uniontown, PA | Full-Time

    Overview

    This is an excellent opportunity for a compassionate and experienced Registered Nurse Case Manager to join our client's hospice team and make a meaningful impact in the lives of patients and families. You will work within a supportive interdisciplinary team, providing essential care coordination, leadership, and patient advocacy across the Uniontown region and surrounding communities.

    Required Qualifications

    Candidates must meet ALL of the following:

    Education & Experience

    • High School Diploma or GED
    • 1+ year of Hospice experience
    • 1+ year of Management or leadership experience
    • 1+ year of experience in ANY of the following:

      • Home Health Care
      • Epic Home Health
      • Home Health

    Licensure & Certifications

    • Valid Registered Nurse (RN) license in the state of Pennsylvania (PA)
    • American Heart Association Basic Life Support (AHA BLS)
    • American Heart Association Advanced Cardiac Life Support (AHA ACLS)

    Additional Requirements

    • Comfortable with the required travel for this position
    • Reliable transportation for daily travel across multiple counties

    Key Responsibilities

    • Manage and oversee patient care in hospice settings
    • Coordinate care plans with interdisciplinary healthcare teams
    • Provide leadership, mentorship, and support to nursing staff
    • Ensure compliance with hospice standards, policies, and state regulations
    • Deliver compassionate, patient‑centered care to individuals and families

    Territory / Driving Expectations

    • Primary territory: Uniontown, PA
    • Travel radius: 15–25 miles surrounding the community
    • Local travel required across multiple counties

    Schedule

    • Full-Time | Monday–Friday | 8:00 AM – 5:00 PM
    • 6 units daily / 30 units weekly
    • On‑Call:

      • 1 evening per week (5 PM – 10 PM)
      • 1 weekend per month
    • Holidays:

      • 2 per year

    Travel

    • Daily local travel required
    • Mileage or travel reimbursement provided (if applicable per client policy)
