JOB SUMMARY: The Hospice RN Case Manger plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interacts with patients, significant others, and other disciplinary team members while maintaining standards of professional nursing and clinical competency. QUALIFICATIONS: Valid license as a registered nurse in the state of Oklahoma. Home health and/or Hospice experience is highly desired. Current BLS certification within 30 days. Minimum one-year of recent professional nursing experience. Having and maintaining a valid Oklahoma drivers license and satisfactory driving record as a condition of employment. Able to cope with emotional stress and be tolerant of individuals lifestyles. Sensitive to the needs of terminally ill patients and families and ones own feelings about dying and death. Basic computer skills required. PHYSICAL REQUIREMENTS: Must have adequate perception of sounds or adequate hearing with corrections. Adequate vision, or correctable with glasses/contacts, to prepare reports and read written materials. Able to speak clearly and distinctly with staff, physicians, patients, and patients families. Must have strength and stamina for prolonged walking, standing, and dealing with stressful situations. Able to hear, or correctable with hearing devices, and able to talk to converse with others in person and over the phone. Adequate vision, or correctable with glasses/contacts, to prepare reports and read written materials. Able to frequently lift at least 60 lbs., reach, pull, push, bend, stoop, and kneel. Possess fine motor skills and hand/eye coordination to give care (i.e.: insert catheters, start IVs, administer medications, and handle instruments). May be exposed to virus, disease, and infection from patients, co-workers, family member and others as well as specimens in working environment. May experience traumatic situations in the family setting.
