Per Diem Nurse Case Manager – Southern California
Rate: $45-52/hour + mileage reimbursement (productive/task time)
Location: Northern California Region (field-based)
Job Type: Per Diem / As Needed


We are seeking a compassionate and skilled Per Diem Nurse Case Manager to join our team in the Southern California region. In this flexible, field-based role, you’ll work directly with injured workers, providers, and employers to coordinate effective care and support return-to-work outcomes.


Key Responsibilities:

• Coordinate and oversee medical care for workers’ compensation cases
  
  
• Communicate regularly with patients, providers, and insurance carriers
  
  
• Develop and manage individualized care plans
  
  
• Support early return-to-work efforts and assist in claim resolution
  
  
• Travel to medical appointments and job sites as needed (mileage reimbursed)
  
  

Requirements:

• Active RN license in California
• Drivers License
• Car Insurance
• Strong clinical background (orthopedic, trauma, or case management preferred)
• Experience in workers’ compensation or case management a plus
• Excellent communication and organizational skills
• Reliable transportation
