RN Case Managers support inpatient discharge planning, care coordination, care progression, post-acute transitions, patient choice compliance, and regulatory requirements.
Current and valid DL or State issued ID required & Utility bill in the Traveler's name with matching address within the last three months. Address must be GREATER than 75 miles from contracted facility.

" Minimum Experience: 2 years as an RN and 1 year as a Case Manager
" Recommended Experience: Recent case/resource management experience
" Required Certification: BLS
" Preferred Experience/Certification: InterQual or MCG experience; CCM or ACM (Certified or Accredited Case Manager) certification through the Case Management Association (CMA) or American Case Management Association (ACMA)
