Requirements

• CA RN License
• BLS Certification
• 5 years of RN Case Management experience
• Inpatient Acute Case Management experience

Responsibilities

• Coordinate care and plan transitions throughout the acute care patient experience.
• Collaborate with physicians, social workers, and other healthcare professionals to ensure timely patient progression and transition.
• Develop cost-effective care coordination plans meeting clinical needs.
• Monitor patient length of stay and manage resources efficiently.
• Formulate transition plans in collaboration with patients, families, and healthcare teams.
• Identify and address barriers to care progression and transition.
• Communicate effectively with financial counselors and make appropriate referrals.
• Contribute to creating cost-effective practices for optimal patient/provider experiences.
