    Registered Nurse Case Manager – Hospice

    Location: Mount Vernon, IL, USA Schedule: Monday–Friday, 8:00 AM–5:00 PM (daily start/end times may vary) On‑Call: 1–2 weekend shifts per month (8A–5P or 5P–8A depending on staffing) Holidays: 2 required per year (1 summer, 1 winter) + 3 floating holidays Travel: Local travel across multiple counties required

You will provide compassionate care, coordinate services, and serve as a clinical leader within a supportive, mission‑driven environment.

    Key Responsibilities

    • Manage and oversee patient care in hospice settings
    • Coordinate care plans with interdisciplinary healthcare teams
    • Provide leadership, guidance, and support to nursing staff
    • Ensure timely and accurate documentation
    • Maintain compliance with hospice standards and regulatory requirements
    • Travel locally to patient homes across multiple counties

    Required Qualifications

    • High School Diploma or GED
    • Valid Registered Nurse (RN) license in the state of Illinois (IL)
    • 1+ year of Hospice experience
    • 1+ year of Management or leadership experience
    • 1+ year of experience in any of the following:

      • Home Health Care
      • Epic Home Health
      • Home Health
    • Valid certifications:

      • American Heart Association Basic Life Support (AHA BLS)
      • American Heart Association Advanced Cardiac Life Support (AHA ACLS)
    • Must not have applied to or worked for Residential Healthcare Group within the last 6 months
    • Must be comfortable with required travel for this position

    Preferred Qualifications

    • RN license number provided at time of application

    Travel Requirements

    • Local travel required across multiple counties
    • Must have reliable transportation and willingness to travel daily
