Location: Mission Hills, CA

Contract Details
- Position Type: Contract
- Contract Duration: 6 months
- Pay Rate: $59.36 $63.36 / Hour (USD)
- Shift / Schedule: Day (5x8 Hours), 8:00 16:30
- Travel Requirements: Not required (No floating expectation)

Job Summary
This travel/contract Registered Nurse Case Manager position is based at an acute care facility in Mission Hills, CA, focused on Care Coordination and Discharge Planning. The role operates within a Case Management department and carries a 1:20 patient ratio. First-time travelers are accepted; clinical screening will be conducted for 1st-time travelers.
Required Skills
- 2-year acute care Hospital Case Manager experience performing Care Coordination and Discharge Planning hospital experience is required
- Ability to perform Care Coordination and Discharge Planning in an acute care setting
- Weekend rotation availability to meet department needs
Education Requirements
- CA State RN License required (pending license accepted, but must be licensed upon start date)
Required Experience
- Minimum 2 years of acute care Hospital Case Manager experience performing Care Coordination and Discharge Planning
- Hospital experience is a must

- RTO Restrictions: No more than 7 days
- Pending CA State License accepted, but candidate must be fully licensed by start date
- Cancellations due to rate changes after signature will result in a systemwide Do Not Return (DNR)
