Description
NEW- Registered Nurse Case Manager
Job Schedule: 8:00 AM - 4:30 PM or 8:30 AM - 5:00 PM, Monday through Friday with rotating weekends and holidays. 13 week contract.
Requirements:
Valid Registered Nurse (RN) license.
Minimum of 1+ years of hospital case management experience
Strong communication and organizational skills.
Ability to work independently and as part of a team.
Proficiency using EPIC EMR system
Job Responsibilities:
Coordinate patient care and ensure continuity of care across various healthcare settings.
Develop and implement individualized care plans for patients.
Collaborate with physicians, therapists, and other healthcare professionals to optimize patient outcomes.
Monitor patient progress and adjust care plans as necessary.
Educate patients and their families about health management and care processes.
Maintain accurate and up-to-date patient records.
