Position Overview

The RN Hospice Case Manager is responsible for coordinating and providing compassionate, skilled nursing care to hospice patients in their homes, assisted living facilities, and other care settings throughout Lehi and the surrounding Utah Valley communities. This role involves managing an assigned patient caseload, collaborating with an interdisciplinary care team, and ensuring patients receive high-quality, individualized end-of-life care that promotes dignity, comfort, and quality of life.

Key Responsibilities

  • Perform comprehensive hospice admissions and ongoing patient assessments
  • Develop, implement, and revise individualized hospice plans of care
  • Coordinate care with physicians, hospice aides, social workers, chaplains, therapists, and other members of the interdisciplinary team
  • Provide skilled nursing care focused on pain management, symptom control, and patient comfort
  • Educate patients and family members regarding disease progression, medications, hospice services, and end-of-life care
  • Monitor patient status and adjust care plans based on changing clinical needs
  • Coordinate necessary medical equipment, medications, and community resources
  • Complete accurate and timely electronic documentation
  • Participate in interdisciplinary team meetings and case conferences
  • Ensure compliance with Medicare hospice regulations and agency policies
  • Provide emotional support and advocacy for patients and their families throughout the hospice experience

Qualifications

  • Current Registered Nurse (RN) license in the State of Utah or eligible for licensure
  • Graduate of an accredited nursing program
  • Minimum of one year of RN clinical experience required
  • Previous hospice, home health, palliative care, oncology, critical care, or case management experience preferred
  • Strong clinical assessment and critical thinking skills
  • Excellent communication, empathy, and interpersonal abilities
  • Ability to work independently while managing a patient caseload
  • Experience with electronic medical records (EMR) preferred
  • Valid driver's license with reliable transportation
  • Current auto insurance meeting state requirements

Education & Certification Requirements

  • Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited nursing program
  • Active Utah Registered Nurse (RN) license
  • Current BLS (Basic Life Support) certification through the American Heart Association or American Red Cross
  • Hospice and Palliative Credentialing Center (CHPN) certification preferred but not required
  • Valid driver's license and acceptable driving record
