Required Qualifications Licensure & Certification
  • Active California RN License
  • Current BLS Certification
Experience
  • Inpatient Acute Care Case Management experience required
  • Complex discharge planning experience required
Key Responsibilities
  • Coordinate patient care across the continuum from admission through discharge.
  • Develop and manage individualized discharge plans for complex patient populations.
  • Collaborate with physicians, nurses, therapists, social workers, and community agencies.
  • Monitor appropriateness and timeliness of care delivery.
  • Evaluate resource utilization and facilitate efficient patient progression.
  • Coordinate transfers to tertiary care facilities and ensure continuity of care.
  • Review treatment plans, diagnostic testing, and discharge needs.
  • Participate in interdisciplinary rounds and care conferences.
  • Identify barriers to discharge and implement solutions.
  • Ensure compliance with regulatory and accreditation standards.
  • Maintain accurate case management documentation.
  • Support patients and families throughout hospitalization and discharge planning.
