Assignment Details
Care Management RN
Schedule: 5x8 Days 8am-430pm, weekdays, some weekends, holidays, no overtime without supervisor approval
Longer assignments through January 9, 2027, are available but not required!!
Travelers must work two of the three winter holidays.
Minimum Requirements
Active Virginia or Compact RN license in hand at the time of submission; no exceptions
Must be proficient in Epic; no exceptions
Important Candidate Requirements
Candidates must meet the following qualifications to be considered:
"Recent experience in an adult acute care hospital setting with discharge planning
"Minimum 5 years of recent acute care Case Management experience (hospital setting)
"Less than a 6-month gap since their last acute care Case Management role
"Experience in a Level I, II, or III acute care hospital
"Previous patient census of approximately 15 25 patients
"Rated 4/5 or 5/5 across all Case Management functions, including knowledge of CMS regulations
"Epic proficiency is required (no exceptions)
"Previous travel experience strongly preferred. First-time travelers will only be considered if they have extensive acute care Case Management experience.
"Acute care discharge planning
"Utilization Review (UR) experience required
"Ability to effectively manage a high-volume, fast-paced patient population
Required Certifications
BLS through the American Heart Association
Preferred Certifications
CCM (Certified Case Manager)
ACM (Accredited Case Manager)
