The Registered Nurse Ambulatory Care (RNAC) is a critical part of the health care team and is responsible and accountable to the health center patients. The health center provides care for acute and chronic care patients from pediatric to geriatrics and has specialty services for obstetric and behavioral health. The RNAC provides care management and coordination to identify high risk patients and assists clinicians in the development of their plan-of-care. In this role the RNAC is responsible for the care management and coordination of a patient panel and act as an advocate, educator, and resource person for this population. Working under the direction of the Center Clinical Manager, the RNAC supports the daily needs of the ambulatory clinic through nursing interventions and execution of medical treatment plans. The RNAC may cover across departments and collaborates with other clinical team members to optimize patient care. Triages acute needs of the patient as well as providing chronic and acute care education, administers medications, and addresses patient and provider taskings. Daily contact with clinicians, health center staff, patients, care givers and outside agencies/providers is required.
Required Education:
Graduate of an accredited School of Nursing.
Preferred Education:
BSN
Required Certifications, Licenses, Registrations:
Registered Nurse licensed in the State of Maryland or Compact State.
BLS
Current MD drivers' license
Preferred Certifications, Licenses, Registrations: None
Required Experience:
Three years clinical nursing experience. In an acute/ambulatory setting
Successful completion of BMS medication and vaccine course
Preferred Experience:
Prior case management experience strongly preferred.
Outpatient clinic nursing experience
