Position: Registered Nurse (RN) Supervisor – Field/Travel
Schedule: Part-Time, Minimum 15 Hours Per Week
Travel: Local and regional travel required; mileage reimbursed
Position Summary
Xclusive Care Services is seeking a dependable and organized
Registered Nurse (RN) Supervisor to conduct required supervisory visits for clients throughout our service areas. This is a field-based position requiring travel from city to city on a
two-week supervisory visit rotation.
The RN Supervisor will be responsible for ensuring assigned supervisory visits are completed within the required two-week timeframe, reviewing the client's current plan of care, evaluating caregiver performance, identifying changes or concerns, and ensuring documentation is completed accurately and on time.
The RN Supervisor will work a
minimum of 15 hours per week, with additional hours potentially available based on the number of clients and geographic coverage needed.
Mileage will be reimbursed for approved business travel.
Key Responsibilities
- Complete assigned client supervisory visits every two weeks according to the agency's required schedule.
- Travel to client homes in various cities and service areas.
- Maintain a minimum workload of 15 hours per week.
- Perform nursing assessments and evaluate clients for changes in condition, safety concerns, and ongoing care needs.
- Review the client's plan of care and verify that services are being provided appropriately.
- Observe and evaluate PCA/CNA/caregiver performance during supervisory visits when applicable.
- Communicate with clients, families, caregivers, nurses, and agency management regarding concerns or changes.
- Identify and promptly report changes in condition, incidents, hospitalization, medication changes, or other significant findings.
- Complete all supervisory visit documentation accurately and within agency-required deadlines.
- Ensure the correct Georgia supervisory visit documentation/forms are completed.
- Review caregiver documentation and report missing, incomplete, or inaccurate documentation when identified.
- Assist with maintaining compliance with agency policies and applicable state, Medicaid, payer, and program requirements.
- Communicate immediately with the Administrator/Nurse Case Manager regarding urgent clinical or compliance concerns.
- Maintain an organized schedule to ensure no assigned supervisory visits become overdue.
- Submit mileage documentation according to agency procedures.
- Protect client confidentiality and follow HIPAA requirements at all times.
Qualifications
- Current and unrestricted Georgia Registered Nurse (RN) license.
- Current CPR/BLS certification.
- Valid driver's license.
- Reliable transportation and ability to travel between multiple cities.
- Current automobile insurance.
- Previous home care, home health, private duty nursing, Medicaid, or supervisory experience preferred.
- Strong assessment and documentation skills.
- Excellent time-management and organizational skills.
- Ability to work independently in the field.
- Comfortable communicating with clients, families, caregivers, and agency leadership.
- Experience with electronic documentation/EVV systems is preferred.
Schedule & Travel Expectations
This position is designed for an RN who enjoys working independently and traveling throughout the community. The nurse will receive a list of assigned clients and will be expected to organize visits efficiently by geographic area.
Minimum Hours: 15 hours per week
Visit Cycle: Supervisory visits completed within each two-week period
Travel: Multiple cities throughout Xclusive Care Services' service areas
Mileage: Reimbursed for approved business-related mileage
The RN must be able to manage their assigned territory effectively and ensure that all required supervisory visits are completed before their deadlines.
