Registered Nurse - ACT Team

We are seeking a nurse interested in working in the community as an ACT Team member servicing individuals with behavioral health needs. 

Location: Manhattan, NY
Schedule: Mon-Fri 09:00am-05:00pm

Compensation:
Pay Range: $50.00-$55.00 Per HR

Role Overview:
• As part of a mobile, multidisciplinary ACT team, the RN will provide comprehensive nursing care and support services to individuals with serious mental illness in community-based settings.

Key Responsibilities:
• Conduct physical and health assessments
• Monitor ongoing health status and coordinate care
• Administer IM injections and manage medications as prescribed
• Provide psychiatric and substance use–related nursing support
• Deliver health education, daily living skills training, and medication management
• Coordinate medical treatment with primary care and community providers
• Support individuals in obtaining stable housing and community resources

Qualifications:
• Active Registered Nurse (RN) license in New York State
• Strong communication and interpersonal skills
• Comfortable working independently in the community
• Proficiency with basic office software and internet research
