GENERAL SUMMARY:
Within the System Capacity Command Center (SC3) at Henry Ford Health, the Access Management Nurse operates under minimal supervision, to review and screen recommended admission cases at Henry Ford Health System Emergency Departments (ED). Utilizes clinical and regulatory knowledge and skills to reduce financial risk and exposure due to inappropriate admissions. Collaborates with ED physicians, attending physicians, and third-party payers regarding initial screening reviews. Facilitates throughput in the most cost-effective manner, through continuous assessments, problem identification, and education.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
Performs the utilization management process using approved clinical screening guidelines to evaluate the recommended admission and may recommend if patients are candidates for observation or inpatient status.
Manages the initial components of the admission process through all access points, establishes relationships that positively impact financial outcomes and ensures complex communication processes occur: a. Reviews orders and communicates with ED physicians to clarify orders that are unclear or contradictory. b. Reviews and provides concise medical information to Physician Advisors when admission order and InterQual output do not align. c. Communicates with third-party payers to ensure notification of patient admission.
Reevaluates ED admissions who have been boarding in the Emergency Department for 18 or more hours to assess potential changes in level of care. Facilitates change in order and retargeting of most appropriate unit based on medical criteria.
Facilitates inpatient transfers from Henry Ford Health freestanding Emergency Department.
Completes documentation according to departmental guidelines.
Identifies, reports and resolves actual and potential delays in service and inappropriate use of resources.
Identifies the most appropriate target unit for patients based on hospital guidelines, admission order, and patient presentation. Communicates with Transfer and Bed Planning teams regarding target unit needs and bed assignments.
Perform other duties as assigned, including acting at times as charge nurse.
Understands and maintains productivity expectations.
Educates healthcare professionals regarding initial screening criteria, recommended patient classification/status, utilization of resources and government regulations that impact the delivery of care.
Supports and collaborates with clinicians to ensure safe and timely transfers.
Participates in the orientation and training of on-boarding SC3 staff.
Participates in activities to promote own continued learning and professional growth.
EDUCATION/EXPERIENCE REQUIRED:
CERTIFICATIONS/LICENSURES REQUIRED:
Licensed as a Registered Nurse in the State of Michigan.
