Registered Assessment Nurse

Hand to Hand Homecare & Transportation is seeking an experienced Registered Nurse (RN) Clinical Supervisor to provide clinical oversight for personal care and respite care services. This hybrid, part-time position involves conducting client assessments, developing individualized care plans, supervising direct care staff, ensuring compliance with state and Medicaid requirements, and supporting quality care delivery.

Qualifications

• Current Virginia RN license or multistate RN privilege.
• Minimum of one year of clinical nursing experience in a qualifying healthcare setting.
• Experience with personal care assessments, care planning, documentation, and staff supervision.
• Strong clinical judgment, communication, organizational, and documentation skills.
• Ability to travel to client locations as needed

Key Responsibilities

• Conduct initial and ongoing client assessments.
• Develop and update individualized care plans.
• Supervise and evaluate aides and LPNs to ensure quality care.
• Complete required clinical documentation and maintain accurate records.
• Perform routine supervisory visits and address client care concerns.
• Participate in quality improvement activities and support compliance with applicable regulations.
• Collaborate with clients, families, caregivers, and agency leadership to promote positive outcomes. 

Work Environment

• Hybrid role involving client home visits, travel within the service area, documentation, and telephone communication.
