    Description

    Job Purpose:

    The individual in this role is responsible for delivering high-quality, compassionate nursing care to patients in their homes. They will conduct supervisory joint visits with Field Nurses, provide training and in-service education to professional clinical field staff and paraprofessional staff, and perform nursing duties as assigned in the Diagnostic & Treatment Center or Medical Practice. This role also serves as a clinical oversight and quality monitoring resource for home-based clinical services.

    Job Responsibilities:

    • Clinical Oversight, Quality Monitoring & Management-Driven Responsibilities
      • Conduct audits and validate clinical documentation to ensure accuracy, consistency, regulatory compliance, and alignment with evidence-based nursing practice.
      • Perform joint supervisory field visits with clinical field staff, observe skills competencies, and provide oversight and supervision to paraprofessional staff.
      • Collaborate with partner organizations, care coordinators, and interdisciplinary team to meet patient needs.
      • Adhere to all clinical protocols, regulatory standards, and infection control guidelines.
      • Supervise, instruct, and guide CHNs, Personal Care Aides, or Home Health Aides.
      • Provide clinical leadership and operational oversight for assigned cases by coordinating care activities, monitoring service delivery, and ensuring alignment with organizational goals, regulatory requirements, and quality standards.
      • Review and monitor clinical documentation, visit completion, and follow-up actions to promote compliance with organizational policies, regulatory standards, and quality benchmarks.
      • Identify trends, barriers, or gaps in care delivery and escalate concerns to leadership while collaborating with interdisciplinary team members to implement solutions that improve patient outcomes and program performance.
      • Cross-trained for Care Delivery Manager Role to provide support and coverage for the Care Delivery Team as required and as assigned.
    • Direct Clinical Responsibilities
      • Perform routine home visits including wound care management, dressing changes, medication administration, and reconciliation.
      • Conduct general health and functional assessments as directed.
      • Provide nursing care in accordance with the individualized care plan, physician orders, and best nursing practice standards.
      • Assess patient conditions and monitor changes in health status, promptly communicating updates to the care team and physician as needed.
      • Educate patients and caregivers on disease management, medication adherence, wound care, and safety in the home environment.
      • Participate in interdisciplinary case conferences and clinical meetings as needed.
      • Perform nursing duties in DTC/Medical practice according to policies and professional nursing scope of practice.
    • Meet a standardized set of competencies for the specific position description established by partner organizations before working independently.
    • Travel between patient homes throughout the designated service area.

    Qualifications:

    • Education: Graduation from an accredited school of nursing is required. Completed NYSED-approved infection control coursework.
    • Experience: Minimum of three years of clinical nursing experience, including 1-2 years in home care or a similar healthcare setting, preferred.
    • Skills:
      • Strong clinical assessment and critical thinking abilities.
      • Excellent communication skills and the capacity to manage multiple participant cases effectively.
      • Minimum of one year of experience working with a frail or elderly population or appropriate training from the organization if less than one year of experience.
    • Licensure: Current active and unrestricted license as a Registered Nurse in New York State.
    • Additional Requirements:
      • Available for weekends/nights/on-call coverage as needed.
      • Proficiency with computers and electronic health record (EHR) systems.
      • Experience with long-term care or managed care systems beneficial.
      • Vehicle and current New York State driver's license required.
      • Legally authorized to practice in the State in which the healthcare professional will perform the function.
      • Medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

    Physical Requirements:

    • Standing duration of up to 6 hours a day.
    • Sitting/stationary positions sedentary position in duration of up to 6-8 hours a day for consecutive hours/periods.
    • Lifting/push/pull up to 50 pounds of equipment, baggage, supplies, and the ability to lift patients safely and using OSHA guidelines.
