The Coordinator, Referral & Placement delivers high-quality and transparent clinical mental health services to fulfill the needs of Diversus Healths clients.

Services include...

  • Addressing crisis calls from community members and individuals whose lives are disrupted or complicated by mental illness, behavioral challenges, substance abuse issues, or difficulties functioning in society.
  • Deliver professional and clinical triage services to individuals facing mental health and substance abuse issues, facilitating dispatch of evaluators, mobile crisis teams, and placement in both external and internal facilities.
  • Collaborate within a multidisciplinary team to provide placement, referral, admission, intervention, or screening, predominantly through telephone communication with occasional in-person interactions. Teamwork is a must.

This position adheres to a fixed, on-site schedule.

Qualifications:

  • Bachelors Degree in a behavioral health or healthcare related field.
  • One year of volunteer or professional experience in the behavioral health field, preferred.
  • A thorough understanding of systems of care and integration of physical and behavioral health.
  • Adapts to differences in organizational cultures (primary care, substance use, social services, etc.).
  • Ability to take initiative and use critical thinking to solve problems independently.
  • Excellent verbal and written communication skills and can easily build rapport.

Position Title: Coordinator, Referral & Placement Location: Colorado Springs, CO (In-person) Employment Type: Full-Time Schedule: 40 hours per week, schedule set by supervisor

Position Summary The Coordinator, Referral & Placement plays a critical role in connecting individuals to life-changing mental health and substance use services. As a key member of our multidisciplinary team, you will manage incoming calls and referrals from individuals in crisis, concerned community members, and partner organizations- ensuring timely triage, placement, and care coordination.

Youll use your clinical judgment and collaborative spirit to support those navigating behavioral health challenges, working closely with internal teams and external providers to provide the right level of care at the right time.

Essential Responsibilities

  • Provide timely and compassionate triage services via phone for individuals facing mental health or substance use crises
  • Coordinate the dispatch of mobile crisis teams and connect individuals with appropriate internal or external resources
  • Serve as a liaison for referrals, admissions, screenings, and placements- ensuring smooth transitions to care
  • Communicate and collaborate with hospitals, emergency departments, law enforcement, and community agencies
  • Maintain accurate documentation of all client interactions in the electronic health record system
  • Support continuity of care and promote safety and stability in crisis situations
  • Work on-site as part of a dedicated and highly collaborative crisis response team

Qualifications

  • Education: Bachelors degree in behavioral health, healthcare, or related field, or Colorado Crisis Professional Certification (required)
  • Experience: One year
  • Knowledge/Skills:
  • Familiarity with systems of care, behavioral health practices, and integrated service models
  • Excellent verbal and written communication; able to build rapport quickly and professionally
  • Ability to take initiative, think critically, and problem-solve under pressure
  • Adaptability to work with diverse populations and collaborate across disciplines
