    Position Summary:

    The Referral Coordinator is responsible for the referral coordination process at a Federally Qualified Health Center (FQHC), ensuring timely, accurate, and efficient patient referrals to external specialists and services. This position serves as a liaison between primary care providers, patients, specialists, and insurance providers. The Referral Coordinator plays a key role in supporting integrated, patient-centered care while ensuring compliance with FQHC and regulatory standards.

    Essential Functions & Responsibilities:

    • Monitor referral processes to ensure timeliness, accuracy, and adherence to policies.
    • Collaborate with clinical teams to clarify referral requirements and ensure appropriate documentation.
    • Track and follow up on pending and incomplete referrals, ensuring continuity of care.
    • Communicate with patients to explain referral processes, confirm appointments, and provide support navigating the healthcare system.
    • Maintain and update referral tracking systems (EHR/EMR), ensuring all relevant data is captured accurately.
    • Generate referral reports and metrics for management; identify areas for improvement.
    • Ensure compliance with HIPAA, FQHC regulations, and payer requirements.
    • Participate in quality improvement projects and interdisciplinary care team meetings.
    • This role manages the requests for patient medical records as well as the release of sed records.
    • Risk adjustment chart reviews are managed by this role
    • Inputs results of received cancer screening metrics from outside referral sources
    • Partners with Chronic Care Management to communication enrolled patients admits and discharges from hospital or skilled nursing facilities
    • Other duties as assigned

    Additional General Requirements: Professional positive attitude, understanding of customer service principles, trustworthiness and excellent interpersonal skills.

    Job Qualifications/Requirements:

    • Education: High School Diploma or GED required; Associate's or Bachelor's degree preferred.
    • Experience: Experience in a community health center, FQHC, or underserved population setting strongly preferred.
    • Prior experience with electronic health records (EHR), preferably eClinicalWorks, Epic, or NextGen
