BRIEF SUMMARY OF PURPOSE OF POSITION: Responsible for assisting with the coordination and planning of the therapeutic and recreational activities for the participant in the Sunrise PACE program. Works under the direct and indirect supervision of the Recreational Therapist and Center Manager or designee. RESPONSIBILITES AND DUTIES: - Implements specific therapeutic programs as part of the plan of care to include recreational and social activities
- Records, signs, and dates progress notes in medical record as overseen by Recreational Therapist
- Assists with developing, implementing, and supervising the activity program
- Assists with the planning of and participation in group and individual activities, including community outings and social events
- Assists with coordinating with skilled nursing or assisted living facilities regarding participant activities and involvement and works with facility staff as needed
- Assists in planning and implementing activities for dementia specific participants
- Assists with developing a calendar of events
- Participates as a member of the Sunrise PACE Interdisciplinary Team as a backup for the Recreational Therapist
- Communicates any changes in participant condition to Recreational Therapist and appropriate team members
- Serves as a resource to staff and coordinates volunteer activities at the Center
- Builds within the Center a sense of community and warm social environment in which participates are able to identify a support system
- Demonstrates active participation in Quality Improvement process
- Coordinates with various community resources
- All other duties as assigned
QUALIFICATIONS AND SKILLS: - High school diploma or GED equivalent
- Minimum 1 - 2 years of experience in a social or recreational program providing and coordinating services for the frail or elderly population within the past 5 years
- Basic computer skills and use of electronic mail.
- Work effectively in a team environment.
- Be empathic and patient to attend to needs of frail elderly population.
- CPR certification or willingness to obtain upon hire
- CNA certification preferred
- Minimum 1 year working with frail elderly population
- Only act within the scope of his/her authority to practice.
- Meet a standardized set of competencies for the specific position description established by Sunrise PACE and approved by regulatory agencies before working independently.
- Possess a valid Michigan driver license and proof of automobile insurance

OSHA CATEGORY: It is unlikely that a person in this position in the regular course of business would encounter tasks or situations involving exposure to bodily fluids. Employee is required to attend Blood-borne Pathogens training on a regular basis as a precaution and to follow all guidelines of said training if an event should ever present itself. PHYSICAL DEMANDS: The duties of this position are consistent with those of a regular clinical work environment as well as those of attending to elderly participants in a home setting.
