Philadelphia FIGHT is now hiring a Records & Referral Specialist! Our Records & Referral Specialists work with patients, providers and other appropriate support to obtain medical records, process referrals and schedule appointments for patients.
About the Role:
- Schedule: Full-time, Monday - Friday (9:00 - 5:00PM)
- Location: Philadelphia FIGHT, 1233 Locust St, 3rd Fl, Phila, PA 19107
- Compensation & Benefits:
- FSLA status is non-exempt with hourly rate of $20.85-$22.00 per hour
- 3 weeks of PTO (4 weeks after 1st year) + 11 paid holidays.

Requirements:
- High school diploma or equivalent required.
- 2+ years' experience in an outpatient primary care medical setting, experience working at an FQHC preferred.
- Must possess basic computer skills to perform job duties including desktop computing, electronic medical records, e-mail, time sheet management, reporting, internet research, etc. using relevant software such as HRIS and Microsoft Word, Excel, etc.
- Discretion, initiative, and ability to maintain HIPAA compliance.
- Due to a high volume of Spanish speaking patients, bilingual candidates (Spanish/English) are strongly preferred.
- Must be able to exercise good judgment and identify when to escalate issues as appropriate.
- Knowledge of de-escalation methods or willingness to be trained in these methods.
- Ability to handle stressful situations while providing exceptional customer service.
- Must be able to demonstrate judgment, patience, integrity, organization, and prioritization in a multi-task environment.
- Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices such as OSHA, HIPAA, HRSA, CLIA and funding body regulations.

Responsibilities:
- Answers and triages multi-line phone calls; retrieves messages and assigns appropriate follow up.
- Obtains medical records and urgent reports from diagnostic exams, specialist consultations and hospitalizations from internal and external sources; maintains updated logins to relevant external healthcare entities as appropriate (i.e., Penn Physician Link, Jefferson Radiology, etc.).
- Assists patients by processing and completing ROI's (request of information) requests.
- Manages incoming documents in the electronic medical records (EMR) system and assigns to rendering care provider, as well as other staff within the health centers.
- Maintains patient files by accurately and timely scanning documents to patients' electronic medical record and conducting regular patient insurance eligibility checks.
- Accurately documents all activities related to patient care appropriately in the Electronic Medical Records/appropriate databases in compliance with contracted funding sources and the established timeframe set forth by FIGHT policy
- Submits prior authorizations for imaging.
- Scrubs schedules and uploads documentation from ER and specialty visits and testing prior to patient visits.
- Closes out all refers as appropriate, including but not limited to backlogs.
- Processes records for Social Security Disability Claims or refers complex cases to outsourced parties.
- Ensures flow of clinical documentation to appropriate work queues for internal and external audits.
- Performs and participates in quality audits and reviews as needed.
- Reviews and disseminates all inbound and outbound faxes and postal mail; follows up as appropriate.
- Releases patient information to appropriate parties in accordance with applicable policies and guidelines.
- Collaborates with Manager - Medical Records & Referrals and/or Nurse Manager to troubleshoot and optimize clinic workflows related to radiology and specialty care referrals.
- Documents and notifies Manager - Medical Records & Referrals of incidents and/or patient complaints.
- Attends daily huddle meetings assigned by manager and discusses care coordination with clinic team.
- Upholds and ensures compliance and attention to all FIGHT policies and procedures as well as the overall mission and values of the organization.
- Other duties as assigned to fulfill FIGHT's mission.
