Title: R-ICMS Community Health Worker Division: HOPICS FLSA: Non-Exempt, Full-Time Pay Range or Rate: $23.00-$26.00 per hour Supervisor: R-ICMS Program Manager
SummaryThe knowledge and skills needed for the Community Health Worker position are typically gained through acombination of lived experience with homelessness or incarceration AND navigating associated servicedelivery systems. For example: parole, probation, community clinics AND six months experience providingpeer support and mentorship to individuals.
Essential Functions• Knowledge of community resources in area of residence• Comfortable working with diverse populations including LGBT, formerlyincarcerated and mentally ill individuals• Exceptional ability to connect and engage with people• Excellent oral and written communication skills• Detail oriented, organized and possess time management skills• Able to work flexible job hours• Willingness to work in various environments, including jail settings, streetoutreach, home visits, homeless encampment• A history of incarceration or homelessness is desirable• Prior experience working with currently and formerly incarceratedindividuals, and/or homeless individuals is desirable• Prior experience as a health navigator, peer support worker, outreachworker or Promotora is desirable• Ability to speak Spanish, or other languages is desirable• Attendance in all mandatory program and division meetings and trainings, as assigned by his/hersupervisor and division director.• Maintain appropriate boundaries; and adhere to SSG's Code of Ethics and HOPICS Core Values.• Represent the Agency in a professional manner at meetings and community events.• Maintain files/records on client services in compliance with HIPAA, 42 CFR Part 2 and other fundingrequirements for audit purposes.• Regular attendance required.• Other duties as needed.
Secondary FunctionsPerform other duties as assigned overtime, holiday, or weekend work may be required.Mini mum Qualifications- Knowledge, Skills and Abilities RequiredValid CA Class C Driver License or access to reliable transportation to carry out job-related essential functionsHigh school diploma or GED. Bachelor's Degree from an accredited college/university desired.Proficiency in Microsoft Word, Excel, and Internet. If in recovery, a minimum of three (3} years of being drug andalcohol free is required. Neat and professional appearance. Attention to detail and punctuality are required; Abilityto communicate effectively, both written and orally required. TB test, CPR and First Aid certification required.

Supervisory ResponsibilitiesThis position does not have any supervisory responsibilities.
Environmental Conditions (Working Conditions)This position in responsible to work in client friendly environments and may be required to visit other sheltersand partner agencies. There is some responsibility to work in noisy environments where children and unstableadults are few to talk loud and expressive.
Physical RequirementsThe staff typically spends time sitting, standing, walking, reading, thinking, note taking, and driving.
Mental RequirementsThis position will require the individual be able to handle any/all the following: constant distractions,interruptions, uncontrollable changes in priorities/work schedules: to be able to process information, think andconceptualize.
