Job Summary
Great Heroes Home Healthcare Agency is seeking a detail-oriented and organized Quality Imporvment Assistant to support our Quality Assurance and Performance Improvement (QAPI), clinical documentation, and regulatory compliance programs. This position plays an important role in maintaining documentation accuracy, tracking quality initiatives, identifying compliance gaps, and supporting continuous improvement. The ideal candidate is highly organized, detail-oriented, comfortable reviewing healthcare documentation, and committed to maintaining high standards of quality and regulatory compliance


Responsibilities 
  • Support the agency’s QAPI program and quality improvement initiatives.
  • Conduct routine reviews of patient records for completeness, accuracy, and timeliness.
  • Track OASIS, Plans of Care, physician orders, visit notes, care coordination, and other required documentation.
  • Identify missing, incomplete, or delayed documentation and communicate findings to leadership.
  • Maintain QAPI, compliance, audit, and documentation tracking tools.
  • Assist with corrective action tracking, follow-up, and performance improvement activities.
  • Support CHAP accreditation readiness, mock audits, and internal compliance reviews.
  • Assist with maintaining organized policies, procedures, compliance files, and regulatory documentation.
Qualifications
  • 1 year in healthcare administration, medical records, quality assurance, compliance, or a similar healthcare support role.
  •  Experience reviewing clinical or patient documentation for completeness and accuracy.
  •  Familiarity with home health documentation, including OASIS, Plans of Care (485), physician orders, visit notes, and care coordination.
  •  Experience with audits, compliance tracking, corrective actions, or quality improvement/QAPI.
  •  Experience working with an EMR/EHR system and maintaining tracking logs and reports.
  •  Strong attention to detail and ability to identify documentation discrepancies and follow up on outstanding items.
  •  Home health, hospic, skilled nuring, or othe post-acute healthcare experience would be a strong advantage.
