Overview
The Age-Friendly Program Manager (AFPM) will be responsible for implementing and sustaining Age-Friendly initiatives across Maimonides Medical Center (MMC). This role will focus on governance structure development, interdisciplinary collaboration, performance tracking, and quality improvement initiatives related to the 4Ms framework (Mentation, Medication, Mobility, and What Matters). The AFPM will lead efforts to strengthen transitions of care, improve post-discharge follow-ups, support Skilled Nursing Facility (SNF) partnerships, and oversee caregiver engagement initiatives as outlined in the AIM 2.0 program.
Responsibilities
The AFPM will act as a liaison between internal stakeholders (e.g., Nursing, Geriatrics, Surgery, Emergency Medicine, Palliative Care, Medical Informatics) and external partners (e.g., SNFs, caregivers, community organizations) to
enhance continuity of care and support patient-centered outcomes.
