Purpose of Your Job Position
The primary purpose of the Qualified Individual Disability Professional (QIDP) is to provide direction, leadership, and supervise the residents’ Individual Program Plan of care. To design, and implement a resident’s treatment care plan catered to the resident’s needs including interventions. The QIDP maintains habilitation management services essential to ensuring a resident’s progress is measured through accomplishing goals set in the Individual program plan.
Duties and Responsibilities
General Functions
- Coordinate and supervise the delivery of each resident’s Individual Program Plan of care (IPP).
- Oversees the delivery of training, and habilitation services of each resident.
- Combines the various aspects of facilities programs.
- Coordinate all efforts of the team to include functioning as a moderator/arbitrator as necessary.
- Maintain effective channels of communication among team members in an effort to expedite the decision-making process to encourage a free exchange of ideas, and productive comments by all team members.
- Prepare monthly reviews in the resident’s record of Interdisciplinary progress notes regarding the response the Residents program plan with written recommendations for continuing, or modifying the IPP.
- Monitors implementation of the IPP and makes appropriate interventions with staff to assure service appropriate to conforming with the IPP.
- Forwards a copy of the Individual Program Plan to send to the resident’s parent/guardian/advocate, and other agencies serving the resident.
- Revises the IPP as needed whenever program changes have been approved by the Interdisciplinary Team.
- Implement behavior intervention programs as written in each Individuals Program plan.
- Train and model for resident’s appropriate behavior intervention techniques.
Personnel Functions
- Determine the staffing needs of the nursing service department necessary to meet the total nursing needs of the residents.
- Recommend the number and level of nursing personnel to be employed.
- Assist the Executive Director and/or the HR Director in the recruitment and selection of nursing service personnel.
- Ensure that all nursing assistants are enrolled in or have graduated from an approved training program.
- Assign a sufficient number of licensed practical and/or registered nurses for each tour of duty to ensure that quality care is maintained.
- Assign a sufficient number of certified nursing assistants for each tour of duty to ensure that routine nursing care is provided to meet the daily nursing care needs of each resident.
- Ensure that all nurse aide trainees are under the direct supervision of a licensed nurse.
- Develop work assignments and schedule duty hours, and/or assist nursing supervisory staff in completing and performing such tasks.
- Delegate to nursing service supervisory personnel the administrative authority, responsibility, and accountability necessary to perform their assigned duties.
- Assist the HR Director in developing performance evaluation schedules, criteria, and annual reviews for the nursing service department (e.g., RNs, LPNs, CNAs, DDCA’s, and medication aides, etc.).
- Make daily rounds of the nursing service department to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards.
- Monitor absenteeism to ensure that an adequate number of nursing care personnel are on duty at all times.
- Develop, maintain, and periodically update the written procedure for ensuring that professional nursing personnel, including private duty nurses, have valid and current licenses as required by this State.
- Review complaints and grievances made or filed by department personnel.
- Provide complaint/grievance reports to the Executive Director as required or as may be necessary.
- Ensure that departmental disciplinary action is administered fairly and without regard to race, color, creed, national origin, age, sex, religion, handicap, or marital status.
- Assist the Infection Control Coordinator in establishing a TB management program for employees.
- Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents.
- Perform background checks on nursing service personnel in accordance with established procedures.
- Ensure that all CNA’s credentials are verified through the State Nurse Aide Registry
Personal Direct Care Functions
- Participate in the interviewing and selection of residents for admission to the facility, • including PASARR screenings.
- Provide the Executive Director with information relative to the nursing needs of the resident and the nursing service department’s ability to meet those needs.
- Inform nursing service personnel of new admissions, their expected time of arrival, room assignment, etc.
- Ensure that rooms are ready for new admissions.
- Greet newly admitted residents upon admission. Escort to room as necessary.
- Make rounds with physicians as necessary. Schedule physician visits as necessary.
- Encourage attending physicians to record and sign progress notes, physicians’ orders, etc., on a timely basis and in accordance with current regulations.
- Ensure that direct nursing care be provided by a licensed nurse, a CNA, and/or a nurse aide trainee qualified to perform the procedure.
- Review nurses’ notes to ensure that they are informative and descriptive of the nursing care being provided, that they reflect the resident’s response to the care, and that such care is provided in accordance with the resident’s wishes.
- Schedule daily rounds to observe residents and to determine if nursing needs are being met in accordance with the resident’s request.
- Monitor medication passes and treatment schedules to ensure that medications are being administered as ordered and that treatments are provided as scheduled.
- Provide direct nursing care as necessary.
- Authorize the use of restraints when necessary and in accordance with our established policies and procedures.
- Ensure that residents who are unable to call for help are checked frequently.
Food Service Functions
- Prepare residents for meals (i.e./ take to bathroom, wash hands, comb hair, raise bed, position tables, place bibs, take to/from dining room, etc.).
- Serve food trays. Assist with feeding as indicated (cutting foods, feeding, assist in dining room supervision, etc.)
- Assist residents with identifying food arrangements (informing resident with sight problems of foods that are on his/her tray, where it is located, and if it is hot/cold etc.).
- Record the resident’s food/fluid intake. Report changes in the resident’s eating habits.
- Serve between meal and bedtime snacks.
- Assist in after meal care (remove trays, clean residents’ hands, face, clothing, take to bathroom, brush teeth, clean dentures)
Staff Development
- Attend and participate in scheduled training and educational classes to maintain current certification as a Nursing Assistant.
- Attend and participate in scheduled orientation programs and activities.
- Attend and participate in facility in-service training programs as instructed.
Safety and Sanitation
- Restrain residents in chair/bed only as instructed by your supervisor. Inform supervisor if resident refuses to be restrained.
- Check restrained residents at least every thirty (30) minutes.
- Release restraints at least every 2 yours for range of motion exercises, taking to bathroom.
- Maintain a record of restrained residents, the times restraints were released, and how long the restraints were released.
- Participate in appropriate in-service training programs prior to performing tasks that involve potential exposure to blood/body fluids.
- Wash hands before and after performing any service for the resident.
- Keep the nurses call system within easy reach of the resident.
- Immediately notify the Nurse Supervisor of any resident leaving/missing from the facility.
- Follow established safety precautions in the performance of all duties.
- Keep residents’ personal possessions off the floor and properly stored.
- Keep floors dry. Report spills immediately.
- Keep excess supplies and equipment off the floor. Store in designated areas.
- Wash wheelchairs, walkers, etc. as instructed.
- Perform routine housekeeping duties (clean bedrails, over-bed table, night stand, that relate to nursing care procedures.
- Report all safety violations.
- Before leaving work, area store all tools, equipment, and supplies.
- Follow established isolation precautions and procedures.
- Wash hands before entering and after leaving an isolation room/area.
- Wear and / or use safety equipment and supplies (back brace, mechanical lifts, etc.) when lifting or moving residents.
- Report missing illegible labels and MSDS to your supervisor.
Equipment and Supply Functions
- Use only the equipment you have been trained to use.
- Operate all equipment in a safe manner.
- Use only the equipment and supplies necessary to do the job. Do not be wasteful.
- Report defective equipment to the Nurses Supervisor.
- Inform the Nurse Supervisor of your equipment and supply needs.
Care Plan Functions
- Review care plans daily to determine if changes in the resident’s daily care routine have been made on the care plan.
- Qualified Intellectual Disability Professional (QIDP)
- Inform the Nurse Supervisor/Charge Nurse of any changes in the resident’s condition so that appropriate information can be entered on the resident’s care
- Ensure that your notes reflect that the care plan is being followed.
Budget and Planning Functions
- Report suspected or known incidence of fraud relative to false billings, cost reports, kickbacks, etc.
Resident Rights
- Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.
- Ensure that resident’s rights to fair and equitable treatment, self-determination, individuality, privacy, property, and civil rights, including the right to wage complaints are followed.
- Knock before entering a resident’s room.

Qualifications and Experience
- Must possess a Bachelor’s degree or higher in a human services field (such as psychology, social work, special education, sociology, or a related human services/professional category).
- Minimum of 1 year of experience working directly with individuals who have intellectual disabilities or other developmental disabilities.
- Must be able to read, write, speak, and understand the English language.
- Must demonstrate the knowledge, and skills necessary to provide care appropriate to the needs of the residents served.
- Must be a supportive team member, contribute to the team, and be an example of teamwork and team concept.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the public.
- Must possess the ability and willingness to work harmoniously with other personnel.
- Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
- Must be willing to seek out new methods and principles, and be willing to incorporate them into existing practices.
- Must be able to relate information concerning a resident’s condition.
- Must not pose a direct threat to the health and safety of other individuals in the workplace.
