Divine Rehabilitation and Nursing of Toledo is looking for an enthusiastic Part time Physical Therapist (PT) to join our team. If you enjoy helping patients regain strength, mobility, and independence—and want the flexibility of PRN scheduling—we’d love to meet you!
What You’ll Do
As a Part time Physical Therapist, you’ll work alongside our interdisciplinary team to provide individualized, patient-centered care. Responsibilities include:
- Evaluate residents and develop individualized treatment plans
- Provide skilled physical therapy services to improve mobility, strength, balance, and function
- Monitor progress and adjust treatment plans as appropriate
- Complete accurate and timely documentation
- Collaborate with nursing, therapy, physicians, and other members of the care team
- Educate residents, families, and caregivers regarding treatment and safety
- Contribute to a positive, team-focused rehabilitation environment
What We’re Looking For
- Graduate of an accredited Physical Therapy program
- Current Ohio Physical Therapist license in good standing
- SNF/LTC experience is a plus
- Experience with Net Health and PointClickCare (PCC) preferred
- Strong communication and documentation skills
- Dependable, compassionate, and team-oriented
