Pay Range: $50.00-$51.00 Per HR
Description:
• Seeking a dedicated professional to provide community-based psychiatric nursing services.
• The role involves onsite work in Philadelphia, PA, with a focus on supporting individuals in their recovery and housing stability.
• The candidate will coordinate care, perform assessments, and engage with a multidisciplinary team. Must have strong communication and organizational skills.
Key Responsibilities:
• Coordinate behavioral and physical health care needs of participants.
• Monitor medication and provide psychiatric nursing services as per standard practices.
• Participate in a team to deliver community treatment services.
• Complete health and biopsychosocial assessments for participants.
• Consult with team members on health care needs and goals.
• Maintain coordination with community agencies and families.
• Document medical history and treatment records promptly.
Requirements:
• Board Certified Licensed RN in Pennsylvania.
• Experience with community mental health facilities.
• Knowledge of psychiatric, clinical, and psychosocial rehabilitation practices.
• Understanding of the Assertive Community Treatment model.
• Strong verbal and written communication skills.
• Organizational and analytical skills.
• Availability for local travel with a valid driver's license.
Qualifications:
• Licensed RN in the state of Pennsylvania.
• Prior experience in psychiatric nursing.
• Familiarity with community mental health practices.
• Ability to work collaboratively in a team setting.
• Proficient in completing assessments and documentation.
