AFM Family & Internal Medicine in Fairfax, VA is looking for a Psychiatric Mental Health Nurse Practitioner to lead behavioral health care inside our primary care practice. This is a chance to build something: you’ll work alongside our physicians and care management team to bring psychiatric care to patients who might otherwise wait months for a referral. If you want to practice integrated, team-based mental health care in a supportive setting, we’d love to hear from you.
Responsibilities:
- Provide psychiatric evaluations, diagnoses and treatment plans for adult patients, many of them older adults with chronic conditions
- Prescribe and manage psychiatric medications, including monitoring, follow-up and coordination with primary care providers
- Serve as the psychiatric consultant for our Collaborative Care (CoCM) and behavioral health integration program, including regular caseload reviews with our behavioral health care manager
- Manage common conditions seen in primary care, including depression, anxiety, ADHD, bipolar disorder, PTSD and substance use concerns
- Offer brief psychotherapy and supportive counseling as appropriate
- Document encounters accurately and on time in our EHR
- Collaborate with physicians, nurses and support staff to keep care coordinated
- Follow up on labs, referrals and patient questions promptly
- Follow all clinical protocols, HIPAA regulations and safety standards
Requirements:
- Master’s or doctoral degree from an accredited PMHNP program
- Current PMHNP-BC board certification through ANCC
- Active, unrestricted Virginia NP license with prescriptive authority, or eligibility to obtain one
- Active DEA registration, or willingness to obtain one
- Psychiatric experience preferred; experience in primary care or integrated behavioral health is a strong plus
- Comfortable with telehealth and EHR systems
- Strong communication skills and a patient-first approach
- Ability to work well in a fast-paced, collaborative environment
This is a remote position.
