About the Role:New Day Health Systems in Catonsville, MD is looking for a passionate and dedicated PRP Worker – Rehabilitation Coordinator to join our team. Responsibilities:- Provide Psychiatric Rehabilitation Program (PRP) services to individuals with mental health diagnoses in community and home settings
- Assist in developing and implementing individualized rehabilitation plans in collaboration with Rehab Specialist and clients and treatment teams
- Coach clients on daily living skills, social skills, and community integration strategies
- Monitor and document client progress toward rehabilitation goals in accordance with program requirements
- Coordinate with case managers, therapists, and other service providers to ensure continuity of care
- Transport and accompany clients to community-based activities and appointments as needed
- Maintain accurate, timely documentation and progress notes in compliance with Maryland COMAR regulations
Requirements:- High school diploma or GED required; bachelor's degree in psychology, social work, or related field preferred
- Prior experience working in psychiatric rehabilitation, mental health, or human services
- Familiarity with Maryland PRP regulations and COMAR standards is a plus
- Strong interpersonal and communication skills with a client-centered approach
- Ability to work independently in community and home-based settings
- Valid driver's license and reliable transportation required
- CPR/First Aid certification or willingness to obtain upon hire
