Responsibilities:

• Develop and implement Individual Service Plans, based on assessments and observations.
• Responsible for 24 hour a day Group Home with rotating on-call and scheduling.
• Supervise, plan, assign, and evaluate or monitor programming and the work of staff responsible for the daily care of individuals.
• Work closely with therapists, service coordinator, administrations, and parents/guardians to maintain communication and provide feedback, standardized procedures, expedite support plan implementation and work-flow, and to improve the efficiency of all care/support providers.
• Performs various assessments; chart reviews; documentation, mealtime and environmental monitoring.

Qualifications:

• BS in Social Work, Social Services, Nursing, Special Education, or related field from an accredited four (4) year college or university.
• 2 or more years'' experience in Program Management in a similar setting
• Proficient in MS Office suites and various industry software programs
• Valid Missouri License with an insurabcle driving record
