CSET is looking for a qualified candidate to join the Youth Workforce Development within the Visalia Navigation Center. The purpose of this position is to develop, coordinate, & facilitate the day-to-day operations of the Homeless Housing, Assistance & Prevention (HHAP) Program focused on individuals aged 18-24 and their families alongside the Department Director/Senior Program Coordinator. Program Coordinators are expected to have a deep knowledge of specific program offerings, perform outreach & recruitment, successfully manage their caseload, develop & maintain relationships with key stakeholders that serve all program goals, & generate reporting to meet projected program requirements. The Program Coordinator will perform ongoing needs assessments to develop personal housing plans, provide support & information, ensure referrals & advocate for individuals. This is a new program & the incumbent will have the opportunity to create & fine-tune the program under the guidance of departmental leadership and to ensure the "housing first" approach is being met.SCOPE OF WORKThe Program Coordinator will support three primary areas of responsibility: Youth Action Board coordination, Rapid Rehousing services, and Project Management. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.ROLE AND RESPONSIBILITES YOUTH ACTION BOARD: The Program Coordinator will coordinate and support the Youth Action Board (YAB), engaging young adults with current or prior lived experience of homelessness or housing instability in meaningful leadership and advisory opportunities.Responsibilities will include recruiting and engaging YAB members, coordinating meetings and activities, supporting leadership development, facilitating participation in community and Continuum of Care activities, and creating opportunities for members to provide feedback and recommendations regarding programs and services.The Program Coordinator will help ensure lived-experience perspectives are incorporated into program planning, evaluation, and continuous improvement.RAPID REHOUSING: The Program Coordinator will provide Rapid Rehousing services for eligible young adults experiencing homelessness with the goal of helping participants obtain and maintain safe, stable, permanent housing.Responsibilities will include intake, assessment, enrollment, development and review of individualized Housing Stabilization Plans, case management, housing navigation, and ongoing housing stabilization support.Housing navigation will include assisting participants with housing searches, rental applications, required documentation, landlord and property manager communication, addressing housing barriers, lease-up, and move-in activities. The incumbent will coordinate allowable housing-related financial assistance in accordance with program requirements and assist participants with financial literacy, household budgeting, income and expense planning, and preparation for maintaining housing after time-limited assistance ends.The Program Coordinator will assess participant needs and connect individuals to appropriate community resources, supportive services, and benefits, including employment, education, transportation, identification documents, healthcare, behavioral health, and other services that support long-term housing stability. PROJECT MANAGEMENT:The Program Coordinator will assist with the coordination, monitoring, and implementation of Youth Action Board and Rapid Rehousing project activities.Responsibilities will include monitoring project budgets and expenditures, maintaining supporting documentation, tracking program outcomes and participant progress, assisting with required reports, and helping ensure activities and expenditures remain within approved funding requirements.The Program Coordinator will maintain accurate participant files, case notes, housing and financial assistance records, YAB documentation, project records, and required HMIS documentation.The position will coordinate with supervisors, fiscal staff, housing providers, community partners, the Continuum of Care, Coordinated Entry, and other stakeholders to support successful project implementation and compliance with CSET policies and applicable grant requirements.Performs other related duties as assigned to support program operations, participant services, and grant objectives. ACCOUNTABILITIES:Maintain an active and engaged Youth Action Board and coordinate meetings, activities, leadership opportunities, and community participation.Ensure Rapid Rehousing participants receive timely assessment, enrollment, housing stabilization planning, housing navigation, case management, and stabilization services.Maintain regular participant engagement and assist individuals in obtaining and maintaining permanent housing.Identify participant needs and connect individuals to appropriate community resources, supportive services, benefits, and referrals.Develop and maintain relationships with landlords, property managers, housing providers, service providers, and community partners.Assist participants with housing searches, applications, documentation, lease-up, move-in requirements, budgeting, and financial planning.Coordinate allowable participant financial assistance and monitor YAB and Rapid Rehousing project expenditures in accordance with approved program requirements.Maintain complete and accurate participant, financial, YAB, project, and HMIS documentation.Track program outcomes, participant progress, housing placements, expenditures, and information required for program and grant reporting.Ensure program activities are carried out in accordance with CSET policies, confidentiality standards, funding requirements, and applicable program guidelines. SKILLS & ABILITIES:Strong case management, participant engagement, communication, organization, and time-management skills.Knowledge of housing navigation, housing stabilization, rental applications, landlord engagement, and barriers to permanent housing.Ability to assess participant needs and connect individuals to appropriate community resources, supportive services, and benefits.Ability to develop and maintain effective relationships with landlords, property managers, community partners, and service providers.Strong facilitation and group engagement skills to support Youth Action Board meetings and leadership development.Ability to assist participants with budgeting, financial literacy, income and expense planning, and housing sustainability.Basic knowledge of project budget monitoring, expenditure tracking, documentation, and grant compliance.Ability to maintain accurate, timely, and confidential participant and program records, including HMIS documentation.Strong problem-solving, conflict-resolution, and decision-making skills.Ability to work independently and collaboratively using a respectful, trauma-informed, Housing First, and participant-centered approach.
