Attend Home Care is seeking a compassionate, dependable, and experienced PRN Registered Nurse (RN) to support our growing home care operations throughout Metro Atlanta and surrounding communities.

We are looking for a motivated nursing professional who enjoys working independently, providing quality care, and making a meaningful difference in the lives of clients receiving services in their homes.

Previous home care experience and familiarity with Georgia Medicaid programs (SOURCE/CCSP) are a plus, but not required!

Key Responsibilities

• Conduct initial nursing assessments and reassessments in clients' homes.
• Develop and update individualized care plans.
• Complete supervisory home visits and client evaluations.
• Assess client needs and ensure appropriate services are provided.
• Maintain accurate, timely nursing documentation.
• Collaborate with caregivers, families, case managers, and agency leadership.
• Ensure compliance with applicable Georgia home care regulations and agency policies.

Qualifications

• Active, unrestricted Georgia RN license.
• Previous nursing experience preferred.
• Home care, home health, or community-based nursing experience is a plus.
• Familiarity with Georgia Medicaid waiver programs, including SOURCE and CCSP, is a plus but not required.
• Strong communication, assessment, and documentation skills.
• Reliable transportation and willingness to travel throughout Metro Atlanta and surrounding areas.
• Ability to work independently and manage a flexible schedule.
