Concord Village Rehabilitation & Wellness is seeking a dependable and motivated PRN Physical Therapist Assistant (PTA) to join our rehabilitation team in Concord, Ohio.

We are currently seeking PRN coverage for both weekdays and weekends, making this a great opportunity for a PTA looking to supplement an existing schedule while making a meaningful impact in a skilled nursing environment.

What You’ll Do

• Provide skilled physical therapy treatments under the direction of a licensed Physical Therapist

• Carry out individualized treatment plans focused on mobility, strength, balance, safety, and functional independence

• Work with residents in a Skilled Nursing/Long-Term Care setting

• Monitor resident progress and communicate changes to the supervising PT

• Collaborate with PTs, OTs, COTAs, SLPs, nursing, and the interdisciplinary care team

• Complete accurate and timely clinical documentation using Net Health

• Provide compassionate, resident-centered care

What We’re Looking For

• Current Ohio Physical Therapist Assistant (PTA) license

• Graduate of an accredited Physical Therapist Assistant program

• SNF/LTC experience preferred

• Familiarity with Net Health is a plus

• Dependable and able to assist with weekday and/or weekend coverage

• Strong communication and teamwork skills

• Compassionate and resident-focused approach to care
