PRN LPN Health Coach - Staffing Support

Under the general supervision of the Office Manager, the Health Coach works independently within the practice and serves as a practice liaison in interactions with other RMG departments to include the remote health care coach team, the Complex Case Managers, and other health care and community services. This practice based health coach must be able to demonstrate effective motivational interviewing techniques as well as demonstrate critical thinking, problem solving and decision making skills to positively impact the optimal health of a patient panel.

Overview
Under the general supervision of the Office Manager, the Health Coach works independently within the practice and serves as a practice liaison in interactions with other RMG departments to include the remote health care coach team, the Complex Case Managers, and other health care and community services. Key duties include a strong focus towards preventive health care as it relates to annual wellness visits and closing care gaps. This practice based health coach must be able to demonstrate effective motivational interviewing techniques as well as demonstrate critical thinking, problem solving and decision making skills to positively impact the optimal health of a patient panel. Float to multiple locations within the division providing support as needed.

What you will do

• Utilizes Practice Dashboard to determine all Medicare eligible patients for Annual Wellness Visits and subsequent AWVs. Assures timely screenings of patients to allow early interventions on positive findings. Provides accurate billing detail on all services performed at the practice to the biller/coder.
• Provides effective education to patients that promotes health and wellness. Provides education and information to the practice team that is timely and relevant to the goals of the medical home.
• Performs pre-visits and patient outreach to assure closure in care gaps. Works quality dashboards to assure care gaps are being addressed. Care documentation is accurate, assuring patient safety. Care concerns from annual wellness visits are addressed timely to provider and/ or office manager. Completes charts with accuracy same day of service.
• Demonstrates the ability to build trusting collaborative relationships with a multi-disciplinary team to support patient care needs. Actively participates in multi-disciplinary team meetings.

Qualifications

Education

• Program Graduate, Licensed Practical Nurse (Required)

Experience

• 1 year Nursing experience (Required)

Licenses and Certifications

• Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) Licensed Practical Nurse Upon Hire(Required) and
• CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) (Required)
