What You’ll Actually Do (The Real Job Description)
- The GPS Lottery: Trust your navigation app implicitly, only to end up on a dirt road staring at a mailbox that doesn't exist.
- Medication Archaeology: Dig through decades of old pill bottles, supplement jars, and expired prescriptions to figure out what the patient is actually taking versus what the hospital discharge paperwork says.
- Wound Care MacGyvering: Clean and dress complex wounds using clean technique while balancing your supplies on a sterilized barrier on top of a kitchen table that is currently holding three puzzles and a cat.
- Pet Negotiations: Politely request that "Button," a 110-pound German Shepherd who is "usually an angel," step into the other room so you can safely check his owner's blood pressure.
- The PRN Ninja Move: Swoop in, complete your visits, document like a boss, and disappear back into your normal life before anyone can ask you to join a committee.

Position Details
- Contingent, per-visit pay structure
- Orientation and additional training compensated at a separate hourly rate
- Competitive wages

Responsibilities
- Conduct initial and ongoing comprehensive patient assessments, including OASIS evaluations
- Complete assessments within 24–48 hours of receiving an initial referral
- Develop, implement, and revise patient care plans in collaboration with physicians
- Provide skilled nursing treatments as outlined in the plan of care
- Educate patients and caregivers using clear, understandable instruction
- Document evaluations, treatments, and patient responses accurately and in a timely manner
- Participate in discharge planning with staff, physicians, and patients
- Attend case conferences and required meetings
- Supervise LPNs in accordance with agency policy
- Ensure compliance with all Federal, State, Medicare, Medicaid, and Accreditation requirements
Minimum Qualifications
- Active Registered Nurse license in good standing in the State of Michigan
- Minimum of two (2) years of nursing experience (home health experience preferred)
- Knowledge of Medicare/CMS guidelines
- Proven experience completing OASIS assessments
- Experience with EMR systems (Kinnser preferred)
- Reliable transportation, valid Michigan driver’s license, and proof of auto insurance
- COVID-19 vaccination required
- BLS certification through AHA or Red Cross
