Responsibilities:- Provide direct, individualized patient care in private home settings according to the established plan of care
- Administer medications, treatments, and therapies as prescribed by the physician
- Monitor and document patient vital signs, health status, and changes in condition
- Communicate effectively with patients, families, and the healthcare team regarding patient progress
- Perform skilled nursing procedures including wound care, catheter care, and IV therapy
- Educate patients and family members on care techniques, medications, and health management
- Maintain accurate and timely clinical documentation and patient records
Requirements:- Active RN or LPN license in the state of Illinois
- Previous private duty, home health, or in-home care nursing experience preferred
- Strong clinical assessment and critical thinking skills
- CPR/BLS certification required
- Excellent communication and interpersonal skills with patients and families
- Ability to work independently with minimal supervision
- Reliable transportation and ability to travel to patient homes in the Stone Park, IL area
