Responsibilities:- Provide direct, individualized nursing care to patients in private duty or home health settings
- Assess, monitor, and document patient health status and changes in condition
- Administer medications, treatments, and therapies as prescribed by the physician
- Develop and implement personalized patient care plans in coordination with the healthcare team
- Educate patients and family members on care procedures, medications, and health management
- Communicate effectively with physicians, specialists, and other healthcare providers
- Maintain accurate and timely clinical documentation and patient records
Requirements:- Active RN or LPN license in the state of Illinois
- Previous experience in private duty nursing, home health, or a related clinical setting preferred
- Strong knowledge of patient assessment, medication administration, and care planning
- CPR/BLS certification required
- Excellent communication, organizational, and critical thinking skills
- Ability to work independently and manage time effectively in a home environment
- Compassionate, patient-centered approach to care
