    PRIMARY CARE PROVIDER (NP/PA)

    Seeking an experienced Primary Care Provider (Nurse Practitioner or Physician Assistant) to provide comprehensive primary care services to an elderly patient population within a collaborative interdisciplinary care model. Working closely with physicians and care team members, the provider will perform assessments, develop care plans, coordinate care, and support positive health outcomes for participants.

    COMPENSATION & BENEFITS

    Pay Rate: $150,000 - $190,000 annually

    KEY RESPONSIBILITIES

    Perform comprehensive initial, semi-annual, routine, and unscheduled participant assessments
    Provide ongoing primary care and clinical management for elderly patients
    Collaborate with physicians and interdisciplinary team members to develop individualized care plans
    Participate in discharge planning from acute care and long-term care settings
    Conduct routine visits and follow-up care in nursing home and community settings as needed
    Coordinate care with physicians, specialists, and interdisciplinary healthcare team members
    Monitor patient outcomes and contribute to quality improvement initiatives
    Review utilization data, hospitalizations, and specialist referrals to support best practices
    Ensure accurate and timely clinical documentation
    Participate in rotating night and weekend call coverage

    REQUIREMENTS

    Master's Degree from an accredited Nurse Practitioner or Physician Assistant program
    Current Nurse Practitioner Certification with Prescriptive Authority or Physician Assistant Certification
    Active state Nurse Practitioner or Physician Assistant License
    Minimum of 2 years of NP or PA clinical experience
    Strong knowledge of chronic disease management and geriatric care principles
    Excellent communication, clinical judgment, and care coordination skills

    PREFERRED EXPERIENCE

    Experience working with geriatric or senior populations
    Experience in primary care, managed care, family practice, internal medicine, or geriatrics
    Experience working within interdisciplinary healthcare teams
    Knowledge of chronic care management and preventative healthcare strategies

    IDEAL CANDIDATE

    Nurse Practitioner or Physician Assistant with at least 2 years of clinical experience
    Experience caring for older adults and medically complex patients
    Strong patient-centered approach to care
    Collaborative team player with outstanding communication skills
    Passionate about preventative care, care coordination, and improving patient outcomes
