We are seeking a Primary Care Physician to join a growing, senior -focused
primary care practice dedicated to delivering high -quality, relationship -based
care for adults age 65 and older. This is an excellent opportunity for a
physician who enjoys practicing medicine in a value -based, managed care
environment with smaller patient panels, longer appointment times, and robust
clinical support.<\/span>

The practice emphasizes personalized care, preventive medicine, chronic
disease management, and strong physician -patient relationships. Physicians are
supported by an experienced care team, allowing them to focus on patient
outcomes rather than high -volume visits.<\/span>

<\/p>- Provide comprehensive primary
care services to Medicare Advantage and senior patients age 65+<\/span>
<\/li> - Perform thorough assessments and
develop individualized care plans<\/span>
<\/li> - Manage chronic conditions,
preventive care, and acute medical concerns<\/span>
<\/li> - Deliver patient -centered care
within a managed care/value -based care model<\/span>
<\/li> - Collaborate with physicians,
specialists, and interdisciplinary care teams to coordinate treatment
plans<\/span>
<\/li> - Identify and close care gaps
while supporting quality and outcome initiatives<\/span>
<\/li> - Educate patients and caregivers
regarding disease prevention, healthy lifestyle choices, and long -term
wellness<\/span>
<\/li> - Build strong relationships with
patients and families through compassionate, personalized care<\/span>
<\/li> - Maintain accurate and timely
documentation within the EMR<\/span>

<\/p>- MD or DO degree from an
accredited medical school<\/span>
<\/li> - Board Certified or Board Eligible
in Family Medicine, Internal Medicine, or Geriatrics<\/span>
<\/li> - Experience in Managed Care,
Value -Based Care, or Medicare Advantage preferred<\/span>
<\/li> - New Graduates Welcome<\/span>
<\/li> - Florida Medical License preferred
or eligible<\/span>
<\/li> - ACN licensed accepted<\/span>
