Compensation: $275,000 – $300,000 + Sign-On Bonus

Position Overview
This is a clinic-based primary care role focused on delivering comprehensive, evidence-based care. You'll work alongside a strong interdisciplinary team to support prevention, chronic disease management, and overall patient wellness.
Responsibilities

• Provide direct patient care in a clinic setting
• Perform Annual Health Assessments (AHA) and Chronic Care Management (CCM)
• Focus on health promotion, disease prevention, and patient education
• Collaborate with interdisciplinary teams to coordinate care
• Utilize EHR systems to manage patient care and documentation

Qualifications

• Board Certified or Board Eligible Physician
• Active CA medical license (or ability to obtain)
• Completed residency in Family Medicine, Internal Medicine, or related field
• 1–3+ years of clinical experience preferred
• Strong communication and teamwork skills
• Experience with EHR systems (Epic preferred)
