Position Summary

We are seeking a Board-Certified Primary Care Physician to join an established outpatient clinic in Glens Falls, New York.

This position offers the opportunity to provide comprehensive primary care services to a Veteran patient population in a collaborative, team-based environment. The physician will work closely with nursing staff, medical assistants, specialists, caregivers, and community resources to deliver high-quality, coordinated care.

The ideal candidate will be passionate about serving Veterans, committed to clinical excellence, and comfortable practicing in an outpatient setting focused on continuity of care and preventive medicine.

Essential Position Responsibilities

  • Provide comprehensive primary care services to adult patients

  • Perform detailed medical histories and physical examinations

  • Diagnose and treat acute and chronic medical conditions

  • Develop and implement individualized treatment plans

  • Order, interpret, and evaluate laboratory tests and diagnostic studies

  • Prescribe medications and monitor treatment effectiveness

  • Provide preventive care and routine health maintenance services

  • Educate patients regarding diagnoses, treatment plans, medications, and lifestyle modifications

  • Assess and address behavioral health concerns when appropriate

  • Collaborate with interdisciplinary care teams to coordinate patient care

  • Review and evaluate outside medical records and specialty consultations

  • Manage chronic disease conditions utilizing evidence-based guidelines

  • Respond appropriately to patient communications, laboratory results, consults, and clinical alerts

  • Participate in urgent care and same-day patient evaluations as needed

  • Complete accurate and timely clinical documentation within established timeframes

  • Maintain compliance with organizational policies, quality initiatives, and regulatory requirements

  • Support patient enrollment, retention, and continuity of care efforts

  • Participate in outreach and community engagement activities when appropriate

  • Contribute to quality improvement initiatives and patient satisfaction goals

  • Deliver exceptional customer service to patients, families, and colleagues

Qualifications

Education

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)

Licensure & Certification

  • Board Certified in Family Medicine or Internal Medicine

  • Active, unrestricted medical license in any U.S. state or territory (subject to contract requirements)

  • Current DEA registration

  • Current Basic Life Support (BLS) certification

  • Current Advanced Cardiac Life Support (ACLS) certification, if required

  • Eligible for credentialing and privileging

Experience

  • Minimum of 4 years of clinical practice experience preferred

  • Previous experience in government, military, VA, or federal healthcare environments is a plus

  • Strong clinical judgment and patient-centered care philosophy

  • Excellent communication and interpersonal skills

  • Experience working within multidisciplinary healthcare teams

  • Proficiency with electronic medical records (EMR/EHR) systems
