    The Opportunity

    Your Role

    • Provide comprehensive primary care to older adult patients

    • Maintain full accountability for clinical outcomes and patient experience

    • Serve as a health coach, guiding lifestyle and long-term health improvement

    • Coordinate specialty and hospital care while staying actively involved

    • Lead and collaborate with an interdisciplinary care team

    • Supervise and remain accountable for Advanced Practice Providers (APPs)

    • Partner with medical and operational leadership to meet quality and cost goals




    Performance Focus (Supported & Measured)

    • Hospital admissions per thousand

    • Appointment utilization (18–21 slots per day)

    • Patient experience (CG-CAHPS)

    • Closure of clinical care gaps

    • Medical cost management




    Qualifications

    • MD or DO required

    • Board Eligible or Board Certified in Internal Medicine, Family Medicine, or Geriatrics

    • Active or eligible state medical license

    • Active DEA

    • Commitment to value-based, team-oriented care




    Compensation & Benefits

    • Salary: $250,000 – $306,714 (based on experience)
